You must be suing someone to use this form.
Note: Some types of cases require additional information. Do not use this summons if you are filing one of these cases:
- Divorce,
- Eviction,
- Small claims, or
- Detinue or replevin.

To complete this form, you will need to know:
- The case number,
- The amount you are suing for (if any),
- The method used for delivering the summons, and
- The name and address of each defendant/respondent.
