This form helps people who owe money to the Illinois Tollway Authority for outstanding toll violations whether or not they have an I-Pass. Use this form to create a payment plan and provide information about your income and bankruptcy disclosures.
- You must have a valid legal reason why you are not responsible for the toll violation, and
- If your name is not on the violation notice, you cannot use this form.
You must have:
- The registered owner first and last name,
- Violation notice number,
- License plate state and license plate number,
- Phone number,
- Valid legal reason (select from options on form), and
- Required documentation for the legal reason.
