A form to appeal a public benefits decision for cash assistance, medical assistance, SNAP, or child care.
You can also appeal a public benefits decision online. If you have an Application for Benefits Eligibility (ABE) account, log in. If you do not have an ABE account, you will need to create one to appeal online. Follow the prompts to link your ABE account to your Integrated Eligibility System (IES) case information and then file the appeal. If you have a Manage My Case Account, log into that account and click on File an Appeal.
Use this form to appeal a public benefits decision for:
- Cash Assistance,
- Medical Assistance,
- SNAP (Food Stamps), or
- Child Care.
To complete this form, you will need:
- The reason you are appealing the original decision, and
- Your case number from the original decision.
