An appellate motion is used to ask the appellate court to do something related to your case.

You must be a party in an appellate case to use this form.
What do I need first?
To use this form, you will need:
- Your name, address, and phone number,
- Your trial and appellate case number,
- The contact information for the parties you want to send the motion to,
- The name and address of the court where the document is being filed, and
- Copies of the Motion and Order for each of the other parties or lawyers.
