People with certain convictions can’t work in a healthcare setting. However, you can get a waiver that removes this barrier. If the waiver is granted, you can be hired even though you have a conviction on your record. Use this form to request a waiver.
To use this form, you must have a criminal conviction that prevents you from working in health care. After you complete the expungement process, you should redo your fingerprints to be removed from the registry after expungement.
To complete this form, you will need to know:
- Your address, telephone number, and social security number,
- Your current and previous employers' names and addresses,
- Your start and end dates for current and previous jobs,
- Your fingerprints from a Livescan fingerprint vendor authorized by IDPH,
- Information about whether your convictions involved alcohol or drugs,
- Information about the conditions of your conviction.
