You can request a Certificate of Good Conduct if you have a criminal record that prevents you from getting a job, work license, or housing. For example, your felony conviction prevents you from driving a school bus, or renting an apartment.
You should first consider expungement and sealing because it may be a better option if you qualify. However, felony convictions can still be seen by some employers, and a Certificate may still be needed.

To use these forms, you need:
- Dates of arrests and charges,
- Outcomes of court cases,
- Addresses of the police departments that filed charges against you, and
- Dates when sentences were completed.
