Use this program when you have a judgment for money and are trying to find out:
- How much the debtor's employer pays the debtor, or
- If the debtor's bank has information about the debtor's property or income.
These forms do not work if you are owed money on a judgment for child support or spousal maintenance.
To complete this program, you will need to know the:
- Case number,
- Courthouse address if court hearing is in person,
- Circuit clerk phone, website, and remote hearing instructions if court hearing is remote,
- Name and address of the debtor,
- Name of the bank or employer,
- Address for registered agent of the bank, or the address for the debtor's employer,
- Date of judgment or the date it has been renewed (revived),
- Amount of judgment, and
- Amount that remains unpaid.
