Complaint for Administrative Review for unemployment Form download
If you want to appeal a Board of Review decision about your unemployment insurance benefits, you will need to file a Complaint for Administrative Review with your local circuit court. Check with your local clerk of court's office to see if they have a Complaint for Administrative Review form. If they do, use your local court's form. If no form exists, you can use this form.
Along with this form, you will also need to complete an Affidavit of Last Known Address and a Summons.

What do I need first?
To complete this form, you will need:
- The names of all necessary defendants, including the director of the Illinois Department of Employment Security (IDES), the Board of Review, and all "parties of record," and
- The decision made by the IDES Board that you want the judge to review.
