County Care Medicaid managed care program

The State of Illinois and Cook County Health & Hospitals System (CCHHS) operate a Medicaid managed care program for uninsured adults in Cook County called CountyCare. This is a Medicaid program through the Affordable Care Act (ACA).

If you sign up for CountyCare, you will get medical care from providers in the CCHHS and others.

CountyCare eligibility

To qualify for a CountyCare Medical Card, you must:

  • Live in Cook County, and 
  • Be enrolled in Medicaid in the state of Illinois.
