A Docketing Statement tells the court that you are at the beginning stage of appealing your case.
To use this form, you must be the appellant in an appellate case. If you filed the Notice of Appeal, you are the appellant. The responding party is called the appellee. The appellant uses the Docketing Statement (Civil).
The deadline to file the Docketing Statement (Civil) is 14 days after you file the Notice of Appeal.
To use this form, you will need:
- Your name, address, phone number, and email,
- Your appellate and trial case number,
- The other party's contact information,
- The Notice of Appeal,
- The final judgment of the trial case, and
- The order about any post-judgment motions.
