Documentation of self-help support/recovery program for DUI Form download
Use this form to show the state of Illinois your participation in a recovery or treatment program.
Can I use this form?
Must have 3 letters of documentation from fellow program members. They must be dated and signed within 45 days of filling.
What do I need first?
In order to provide documentation of self-help support/recovery program, fellow program members will answer the following:
- How long have you known the petitioner,
- How long you have personally known the petitioner to be involved in the program, and
- How often do you see the petitioner at the program (daily, weekly, monthly, etc.).
You will also need your:
- Name,
- Driver's license number, and
- Address.
