Use this form to get an abstract of your driving record.
Can I use this form?
You must be able to select a legal reason why you need an official abstract. Select from the boxes on page 2 of the form.
What do I need first?
- Name,
- Drivers license number,
- Date of birth,
- Gender,
- Reason for request, and
- Notary if you are mailing the form.
If you are a representative or agent of anyone other than yourself, you must provide:
- Name of person or organization you are representing,
- Address, and
- Notary if you are mailing the form.
