You must have one of the following relationships to the child or children:
- Grandparent or great-grandparent,
- Sibling or half-sibling, or
- Step-parent.
You must have been unreasonably denied visitation with the child or children.
You will need:
- Child or children's names and dates of birth,
- Parents' names, and
- County where the child or children live now.
