Use this form to ask an Illinois court to enforce or modify a divorce or custody judgment from another state.
To complete this form, you will need a certified copy of the original judgment. You can get a certified copy from the clerk in the county where the court entered the original judgment.
Attach the original judgment to this form and mark it as "Exhibit A."
