To use this program, you must have one of the following types of unpaid wages:
- Unpaid regular wages,
- Unpaid overtime wages,
- A minimum wage violation,
- Unpaid vacation pay,
- Unpaid bonus pay,
- Unpaid commission, or
- Illegal deductions.
You must also create an Illinois Public ID if you do not have one yet.
To complete this program, you will need:
- Your employer's name, address, and phone number,
- The name of the person responsible for paying employees,
- The number of hours that you worked but were not paid,
- The dates you worked and how many hours per week for minimum wage and overtime claims
- The amount of vacation pay, bonuses, commissions, or illegal deductions that you are owed,
- The dates you took off as paid vacation days for vacation claims, and
- The employer's policies for vacation pay, bonuses, and commissions.

Payroll deductions have to be agreed to in writing by the employee, regardless of whether the employee is exempt or non-exempt to be legal deductions in Illinois. Hope this helps!
