This form will help you certify that you are experiencing homelessness so you can apply for a state ID card for free.
Get the form notarized before you apply at the Illinois Secretary of State’s office. Use the form no later than 90 days from the date it was notarized. Only a shelter or public service agency can verify that you are homeless.
This form does not establish proof of identity and does not take the place of a state ID card.
To use this form, you must be experiencing homelessness and a shelter or public service agency can verify that you are receiving services.

- Your name, date of birth, and signature,
- Signature of a notary republic, and
- Contact information and notarized signature of agency you are receiving services from.
