Easy Form update in progress
Note: As of January 1, 2024, there are new name change forms in Illinois. ILAO is updating our Easy Form to make the new forms.

To use these forms, all of the following must be true:
- You must be 18 or older,
- You must have lived in Illinois for at least 6 months, and
- If you have ever been convicted of identity theft or sexual offenses, you must have received a pardon for all of those offenses.
If your spouse is also changing their name, they must also meet these requirements.
If the person requesting a name change has ever been convicted of identity theft, a sexual offense, or a felony, they will need to know:
- The date of conviction or probation for each offense,
- The sentence you received,
- The date the sentence was completed, and
- Whether the offense was pardoned by the governor.
